Anaphylaxis Forms

All applicable forms must be signed by the parents of cadets heading to CTC this summer. One important caveat that must be clearly articulated: on the Anaphylaxis Emergency Plan form, there is no requirement for a physician to sign the form for this summer, nor is there a requirement for parents to provide a picture. 

What’s included in the attached files:

1.       Cover letter signed by Comd. This letter is to inform parents as to why the new forms are required.

2.       Anaphylaxis Consent Form. To be filled out by any cadet who is required to carry an epi-pen.

3.       Participation by a Cadet with Non-Food Related Anaphylaxis. To be filled out if cadet has non-food related anaphylaxis (ie allergies to bee stings).

4.       Anaphylaxis Emergency Plan. To be filled out for any cadet who has one or both of the forms filled out listed above. For the 2017 CTC season, a signature by a physician is not required, nor is a photo.

5.       Participation by a Cadet with Food Sensitivity. To be filled out by any cadet who has a food sensitivity (food intolerance, chemical sensitivity, food allergy, life threatening food allergy (anaphylaxis). If the form is signed due to anaphylactic reaction to some food, the Anaphylaxis Emergency Plan must be filled out.
